Many of my foreign clients come to insurance consultations in Spain without a clear understanding of how the Spanish healthcare system works. Naturally, they start by comparing it to the insurance products available in their home countries. And while this comparison is understandable, healthcare and insurance in Spain operate under very different principles. In many countries, insurance companies offer separate maternity packages or standalone pregnancy insurance plans. In other words, pregnancy and childbirth are treated as an independent commercial insurance product, purchased specifically for this life stage. Spain works differently.
Historically, Spain has one of the oldest and most comprehensive public healthcare systems, where pregnancy and childbirth have long been considered a social priority and a fundamental right. Because of this, private health insurance in Spain was built on top of already established public healthcare principles.
As a result, pregnancy and childbirth are not insured through separate policies, but are instead included within general private health insurance, provided certain conditions are met.
Private health insurance for pregnancy in Spain offers future parents important advantages, such as:
- the freedom to choose a doctor and clinic
- more frequent and detailed medical check-ups
- greater consideration of the mother's personal preferences
Today, many families choose private health insurance for pregnancy and childbirth in Spain in order to receive a higher level of comfort, flexibility, and personalized care — benefits that the public healthcare system cannot always provide.
Private Health Insurance Before Moving to Spain
For foreigners who are not yet living in Spain and planning a pregnancy, private health insurance can play an important role during the first months after arrival.
Administrative processes such as:
- residency approval
- access to the public healthcare system
- obtaining social security coverage
often take time and cannot be arranged in advance or online from abroad.
In addition, it is important to understand that not all types of residence permits in Spain provide access to public healthcare. For many foreigners, access to the public system depends on employment status, social security contributions, or the specific type of residency they hold.
In contrast, private health insurance can be arranged online before moving to Spain, while still living in another country, and activated from day one — regardless of the type of residence permit.
Having a private health insurance policy arranged in advance allows future parents to:
- access doctors and clinics immediately after arrival
- avoid gaps in medical care during the relocation period
- feel more secure during an important and sensitive stage of life
This is especially relevant for couples who are planning pregnancy while relocating to Spain.
